Summary
Embryo transfer (ET) has been recognized as a vital step which influences pregnancy rates in patients undergoing the in vitro fertilization process (IVF). The safe placement of embryos inside the uterine cavity is essential to obtain an optimal pregnancy rate. However, there is no guarantee that the embryos will remain in the uterine cavity after the procedure. Embryos have been found in catheters, on the cervix, and on the vaginal speculum. The performance of an atraumatic ET is essential to IVF success. The other factor influencing ET are contamination of the catheter with blood, mucus or endometrial tissue, as well as the occurrence of retained embryos. Embryo retention in the catheter is a common phenomenon, encountered by even the most experienced operators. Embryo retained in the transfer catheter or its sleeve require repeat transfer. Pregnancy rates after IVF-ET decrease in a stepwise fashion with the increasing frequency of uterine contractions. To achieve state of maximal uterus relaxation during embryo transfer it is advised to perform ET gently without excessive stimulation of the cervix. Despite revolutionary changes in the embryological laboratory, little has changed with process of ET over past 20 years. Even thought, ET seems to be relatively easy procedure it should be performed by the most experienced operators.



Transfer zarodków (ang. embryo transfer – ET) stanowi integralny i kulminacyjny element leczenia metodami zapłodnienia pozaustrojowego. Jego głównym celem jest bezpieczne umieszczenie zarodków o jak najwyższym potencjale rozwojowym w jamie macicy. Pomimo pozornej prostoty poprawne wykonanie ET jest dość trudne i wymaga sporego doświadczenia. Spośród wielu czynników mogących mieć wpływ na powodzenie transferu najczęściej zwraca się uwagę na czynniki związane z anatomią i fizjologią układu rozrodczego oraz na aspekty techniczne ET.
Łatwość wykonania transferu ma zasadniczy wpływ na ostateczny wynik leczenia. Z obserwacji klinicznych wynika, że prawdopodobieństwo uzyskania ciąży jest znacząco większe w przypadku transferu łatwego niż trudnego, tzn. przebiegającego z komplikacjami (1). W trakcie przechodzenia kateterem przez kanał szyjki macicy może dojść do uszkodzenia naczyń krwionośnych. Obecność krwi na kateterze po wykonaniu ET jest uznawana za oznakę trudnego transferu i czynnik zmniejszający szansę na powodzenie leczenia (2). Z drugiej strony istnieją badania wskazujące na to, że obecność krwi na kateterze nie ma wpływu na wskaźnik implantacji i uzyskanych ciąż klinicznych (3).
Dość częstą przyczyną trudnych transferów są warunki anatomiczne związane ze zbyt silnym przodo-, tyłopochyleniem lub przodo-, tyłozgięciem macicy, jak również ze zwężeniem kanału szyjki macicy (4). Zwiększa się wtedy ryzyko uszkodzenia naczyń krwionośnych kanału szyjki macicy. W takich przypadkach korzystne wydaje się być wykonanie transferu próbnego lub przeprowadzenie badania ultrasonograficznego (USG) w trakcie ET.
Oblepienie kateteru śluzem z gruczołów szyjki macicy podczas przechodzenia nim przez kanał szyjki może spowodować przytkanie wylotu kateteru i utrudnić wstrzyknięcie zarodka do jamy macicy (5). Ponadto zarodek w czasie wstrzykiwania może przykleić się do śluzu oblepiającego kateter i zostać wyciągnięty wraz z kateterem po zakończeniu ET.
W badaniu przeprowadzonym na ponad tysiącu cykli wykazano jednak, że szanse na uzyskanie ciąży były obniżone jedynie w przypadkach, w których obecności śluzu na kateterze towarzyszyła obecność krwi (6). Istnieją także badania wskazujące, że usuwanie śluzu z kanału szyjki macicy przed ET nie ma istotnego wpływu na wyniki transferu (7).
